    Spatial variations in COVID-19 risk perception and coping mechanism in Pakistan

    The outbreak of novel coronavirus disease (COVID-19) was declared a pandemic by the World Health Organization, which instigated governments to impose lockdowns across their countries. Amidst the lockdown in Pakistan, this study comprised measures of the COVID-19 risk perception, coping mechanism, and spatial variations. The data from 40 selected indicators was collected using an online questionnaire and grouped into domains (4 risk perception and 3 coping mechanisms domains). The results revealed the spatial variations and the levels of risk perception and coping mechanisms within the study area. Relative to each other, overall risk perception was highest in Northern Areas (Gilgit-Baltistan and Azad Jammu and Kashmir) and Islamabad, and lowest in Balochistan province. Very little spatial variation was observed in terms of coping mechanisms. Age, gender, and marital status influenced the risk perception associated with COVID-19. The findings suggest spatial variation in risk perception, implying the need for localized and modified COVID-19 risk communication and risk reduction strategies.     An Analysis of the Relationships among Exports, Imports, Physical Capital and Economic Growth in Pakistan

    This review emphasized the relationship among capital formation, economic growth, exports and imports in case of Pakistan scenario using time series data from 1976 to 2015. Augmented Dickey Fuller Test, Johansen Co-integration, Vector error correction model and Granger Causality techniques have been used to check the relationships among exports, imports and economic growth. The results from this study show that the exports, imports, real GDP and gross fixed capital formation have a long run relationship and are co-integrated. This study uses the data of Pakistan and concludes that GDP doesn’t granger cause with the export and import while export and imports do granger cause with the GDP in the long run. Finding of the study also displays that physical capital formation has no impression over GDP. Previous study shows the positive relation among exports, imports, capital formation and economic growth while this study shows that in the long run capital formation and economic growth has no effect. Government subsidizes the exports and also increases the duty bills on imports that help boost the domestic industries manufacture the goods and motivate to produce the best quality of goods.     Relationship of Residential Location Choice with Commute Travels and Socioeconomics in the Small Towns of South Asia: The Case of Hafizabad, Pakistan

    The existing literature of emerging markets fails to provide evidence to clarify if people choose their residential location based on commuting to work or other socioeconomic or household factors. The present paper seeks to provide such evidence in South Asia using the case study of a small city in Pakistan. This exploratory study was facilitated by primary data collected from 365 adults in Hafizabad, Pakistan, using face-to-face interviews in 2018. Two research questions were answered: (1) with what socioeconomic or mobility-related variables are the residential self-selections correlated? (2) how strong is the possible association of commuting to work to residential location choices compared to other factors, including social, economic, and family-related issues? The results of Chi-square tests and Proportional Reduction in Error analyses show that the three variables of neighborhood place, gender, and housing tenure type are associated with residential location choices. These findings are partly in line with studies on high-income countries, but gender and housing tenure are more specific to developing countries. Moreover, results of a Binary Logistic model show that marital status and house ownership of other household members define whether people choose their living place based on commuting rather than other socioeconomic and household issues. The finding of the latter variable contrasts with behaviors in high-income countries, whereas the former variable has some similarities.     An Integrated Approach of Machine Learning, Remote Sensing, and GIS Data for the Landslide Susceptibility Mapping

    peer reviewedLandslides triggered in mountainous areas can have catastrophic consequences, threaten human life, and cause billions of dollars in economic losses. Hence, it is imperative to map the areas susceptible to landslides to minimize their risk. Around Abbottabad, a large city in northern Pakistan, a large number of landslides can be found. This study aimed to map the landslide susceptibility over these regions in Pakistan by using three Machine Learning (ML) techniques, specifically Linear Regression (LiR), Logistic Regression (LoR), and Support Vector Machine (SVM). Several influencing factors were used to identify the potential landslide areas, including elevation, slope degree, slope aspect, general curvature, plan curvature, profile curvature, landcover classification system, Normalized Difference Water Index (NDWI), Normalized Difference Vegetation Index (NDVI), soil, lithology, fault density, topographic roughness index, and road density. The weights of these factors were calculated using ML techniques. The weightage overlay tool is adopted to map the final output. According to three ML models, lithology, NDWI, slope, and LCCS significantly impact landslide occurrence. The area under the ROC curve (AUC) is applied to validate the performance of models, and the results show the AUC value of LiR (88%) is better than SVM (86%) and LoR (85%) models. ML models and final susceptibility map gives good accuracy, which can be reliable for the results. The study’s outcome provides baselines for policymakers to propose adequate protection and mitigation measures against the landslides in the region, and any other researcher can adopt this methodology to map the landslide susceptibility in another area having similar characteristics

    Antihyperlipidaemic Efficacy of Trachyspermum ammi in Albino Rabbits

    The objective of the present study was to investigate the antihyperlipidaemic efficacy of Trachyspermum ammi (L) Sprague, seed (Ajowan) powder in albino rabbits. Hyperlipidaemia was induced with butter ad libitum and oral intubation of cholesterol 400 mg/kg body weight. Simvastatin, (Tablet survive®) was used as a synthetic cholesterol lowering drug. The results suggest that 2 g/kg T. ammi seed powder produced hypolipidaemic activity, rendering 49%, 53%, 71% and 63% reduction in total lipids, triglycerides, total cholesterol and LDL-cholesterol, respectively. However, at this dosage level 62% increase in the value of HDL-cholesterol was induced by T. ammi seed powder. Nonetheless, 0.5 g/kg and 1 g/kg T. ammi seed powder did not show antihyperlipidaemic activity. T. ammi seed powder at the rate of 2 g/kg and simvastatin (0.6 mg/kg body weight) were equally effective in treating hyperlipidaemia in albino rabbits. Moreover, this dosage level also significantly reduced the cholesterol content of liver tissue. It can be anticipated that the lipid lowering mechanism may involve enhanced removal or catabolism of lipoproteins, inhibition of HMG COA reductase, and/or inhibition of lysosomal lipid hydrolytic enzymes secreted by the liver

    Knowledge, attitudes, and practices among nurses in Pakistan towards diabetic foot

    Introduction: Diabetic foot ulcers are a pressing complication of diabetes mellitus. Wound care requires a significant proportion of healthcare resources. It is imperative, therefore, for healthcare professionals to possess sound knowledge of the disease along with a positive attitude to ensure better clinical practice. Our literature search revealed a scarcity of data pertaining to diabetic foot ulcers. Therefore, this study aims to evaluate the knowledge and attitudes of nurses regarding diabetic foot care. Methods: A cross-sectional study design was employed, a pre-validated and pre-tested questionnaire was used to collect data from a sample size of 250 nurses working at two tertiary care hospitals in Karachi, Pakistan. The study was conducted over a period of three months (January to March 2018) and included all nurses who possessed at least one year of clinical experience in diabetic ulcer care. The statistical software employed was SPSS version 19 (IBM Corp., Armonk, NY, US). Non-parametric tests and descriptive statistics were used for data analysis and statistical significance was assumed at a p-value of less than 0.5. Results: Only 54% of the nurses in our study possessed adequate knowledge of diabetic foot ulcers. The mean score of knowledge was 74.9 (±9.5). Macdonald’s standard criteria for learning outcomes was used to gauge the knowledge levels of our study population. Nurses performed best in the domain of ulcer care with 65.3% of the participants possessing good knowledge of the topic. The overall attitude of nurses towards patients with diabetic ulcers was positive. Conclusion: This study highlights important gaps in nurses’ knowledge and sheds light on the lack of evidence-based practice. Poor knowledge can compromise healthcare standards, even with the presence of positive attitudes.     Transition metal doped CeO2 for photocatalytic removal of 2-chlorophenol in the exposure of indoor white light and antifungal activity

    Besides natural sunlight and expensive artificial lights, economical indoor white light can play a significant role in activating a catalyst for photocatalytic removal of organic toxins from contaminated water. In the current effort, CeO2 has been modified with Ni, Cu, and Fe through doping methodology to study the removal of 2-chlorophenol (2-CP) in the illumination of 70 W indoor LED white light. The absence of additional diffractions due to the dopants and few changes such as reduction in peaks’ height, minor peak shift at 2θ (28.525°) and peaks’ broadening in XRD patterns of modified CeO2 verifies the successful doping of CeO2. The solid-state absorption spectra revealed higher absorbance of Cu-doped CeO2 whereas a lower absorption response was observed for Ni-doped CeO2. An interesting observation regarding the lowering of indirect bandgap energy of Fe-doped CeO2 (∼2.7 eV) and an increase in Ni-doped CeO2 (∼3.0 eV) in comparison to pristine CeO2 (∼2.9 eV) was noticed. The process of e-– h+ recombination in the synthesized photocatalysts was also investigated through photoluminescence spectroscopy. The photocatalytic studies revealed the greater photocatalytic activity of Fe-doped CeO2 with a higher rate (∼3.9 × 10−3 min-1) among all other materials. Moreover, kinetic studies also revealed the validation of the Langmuir-Hinshelwood kinetic model (R2 = 0.9839) while removing 2-CP in the exposure of indoor light with a Fe-doped CeO2 photocatalyst. The XPS analysis revealed the existence of Fe3+, Cu2+ and Ni2+ core levels in doped CeO2. Using the agar well-diffusion method, the antifungal activity was assessed against the fungus M. fructicola and F. oxysporum. Compared to CeO2, Ni-doped CeO2, and Cu-doped CeO2 nanoparticles, the Fe-doped CeO2 nanoparticles have outstanding antifungal properties

    Burnout among surgeons before and during the SARS-CoV-2 pandemic: an international survey

    Background: SARS-CoV-2 pandemic has had many significant impacts within the surgical realm, and surgeons have been obligated to reconsider almost every aspect of daily clinical practice. Methods: This is a cross-sectional study reported in compliance with the CHERRIES guidelines and conducted through an online platform from June 14th to July 15th, 2020. The primary outcome was the burden of burnout during the pandemic indicated by the validated Shirom-Melamed Burnout Measure. Results: Nine hundred fifty-four surgeons completed the survey. The median length of practice was 10 years; 78.2% included were male with a median age of 37 years old, 39.5% were consultants, 68.9% were general surgeons, and 55.7% were affiliated with an academic institution. Overall, there was a significant increase in the mean burnout score during the pandemic; longer years of practice and older age were significantly associated with less burnout. There were significant reductions in the median number of outpatient visits, operated cases, on-call hours, emergency visits, and research work, so, 48.2% of respondents felt that the training resources were insufficient. The majority (81.3%) of respondents reported that their hospitals were included in the management of COVID-19, 66.5% felt their roles had been minimized; 41% were asked to assist in non-surgical medical practices, and 37.6% of respondents were included in COVID-19 management. Conclusions: There was a significant burnout among trainees. Almost all aspects of clinical and research activities were affected with a significant reduction in the volume of research, outpatient clinic visits, surgical procedures, on-call hours, and emergency cases hindering the training.     Impact of opioid-free analgesia on pain severity and patient satisfaction after discharge from surgery: multispecialty, prospective cohort study in 25 countries

    Background: Balancing opioid stewardship and the need for adequate analgesia following discharge after surgery is challenging. This study aimed to compare the outcomes for patients discharged with opioid versus opioid-free analgesia after common surgical procedures.Methods: This international, multicentre, prospective cohort study collected data from patients undergoing common acute and elective general surgical, urological, gynaecological, and orthopaedic procedures. The primary outcomes were patient-reported time in severe pain measured on a numerical analogue scale from 0 to 100% and patient-reported satisfaction with pain relief during the first week following discharge. Data were collected by in-hospital chart review and patient telephone interview 1 week after discharge.Results: The study recruited 4273 patients from 144 centres in 25 countries; 1311 patients (30.7%) were prescribed opioid analgesia at discharge. Patients reported being in severe pain for 10 (i.q.r. 1-30)% of the first week after discharge and rated satisfaction with analgesia as 90 (i.q.r. 80-100) of 100. After adjustment for confounders, opioid analgesia on discharge was independently associated with increased pain severity (risk ratio 1.52, 95% c.i. 1.31 to 1.76; P < 0.001) and re-presentation to healthcare providers owing to side-effects of medication (OR 2.38, 95% c.i. 1.36 to 4.17; P = 0.004), but not with satisfaction with analgesia (beta coefficient 0.92, 95% c.i. -1.52 to 3.36; P = 0.468) compared with opioid-free analgesia. Although opioid prescribing varied greatly between high-income and low- and middle-income countries, patient-reported outcomes did not.Conclusion: Opioid analgesia prescription on surgical discharge is associated with a higher risk of re-presentation owing to side-effects of medication and increased patient-reported pain, but not with changes in patient-reported satisfaction.
